Street ruffian Lin San is forced to take on the dangerous task of "curing the royal family's secret illness" after accidentally saving the dying head of the Imperial Medical Bureau. With his cunning tricks and self-taught "unorthodox" medical skills, he reluctantly breaks into the palace—facing刁难 from stuffy old scholars in the Imperial Medical Office, solving the mysteries of the Empress Dowager's chronic illness and the Crown Prince's sudden strange disease, and even getting tangled in the Concubine's power-seizing plot. What's more, Ah Zhao, the little maid he saved as a child, is now the "living abacus" beside the Empress Dowager, always catching him for his ruffian habits. When ruffianism clashes with royal rules, and unorthodox medicine meets palace secret recipes, can Lin San survive the treacherous palace and rise to become the legendary "First-Rank Imperial Physician"?
